If your child is covered under an insurance plan, we will happily file dental claims to your insurance company. Complete insurance information must be presented at the time services are provided and updated yearly or in the event of a change in policy information.

Are you in network with my insurance?
At Fishers Pediatric Dentistry we are a participating provider with the following companies: Aetna PPO, Anthem Dental Blue (100, 200, 300), Anthem Dental Complete, Children’s Special Health Care Services, Delta Dental PPO, Delta Dental Premier, Guardian PPO, and Indiana’s Medicaid and Hoosier Healthwise.
What does my insurance cover?
It is important to understand that all dental insurances are different and coverage varies depending on your plan. We will always do our best to estimate what your payment portion will be. However, this is just an estimate and not a guarantee your insurance company will pay what we estimate they will. In the event that your insurance has not paid your account within 60 days, the balance may be transferred to your account, and must be paid within 15 days.
You may contact your insurance directly to verify your specific plans' coverage for common procedures: Fluoride Varnish (Code D1206), Sealants (Code D1351) or Panoramic X-rays (Code D0330)
What kind of payment options do you have?
Our practice offers a wide range of payment options. Our Treatment Coordinator will work out a plan suited to your individual needs. For your convenience, we accept cash, checks, Visa, MasterCard, American Express, and Discover. For patients with significant treatment needs, we can help you set up a payment plan with Care Credit.
CareCredit® is a health care card that is exclusively utilized for dental and medical services. They offer flexible payment options that fit your timetable and budget.
